We are less than 3 miles away from the US-60 and within a 10 minute drive from several major hospitals, medical facilities, and doctors´ offices. 24th Street is directly between Gilbert Rd. & Lindsay Rd. with the cross-street being Southern Ave.

What is required from you
When you have made the decision to move your loved one into our facility, you will need to inform theirr primary care physician. The facility manager will provide the physician with the necessary admissions paperwork stating your loved one is appropriate for an assisted living.
The resident must be administered a tuberculosis test, as well as a COVID-19 test showing a negative result prior to moving into the facility. If available, you will need to provide the facility with documentation of Durable Power of Attorney and Living Will stating DNR status. The first month’s rent and administration fee are due upon admission.
